What Does a Functional Nutrition Assessment Identify?

From the medical history, physical and laboratory exams, and a diet and lifestyle review, the Functional Nutrition Assessment improves the identification of insufficiencies/imbalances through the PFC-MVP model, representing Protein, Fat/ Oils, Carbohydrates, Minerals, Vitamins, and Phytonutrients.

Nutrition Consult & Follow up

During the initial consultation, we will conduct a 90 minute Nutrition assessment of your medical, dietary, and medication history, and diagnostic and lab test results. Based on this information obtained, we will work with you during the  included 45 minute Nutrition follow up to create a personalized meal plan based on budget & taste preferences along with lifestye goals .  If you feel you would like  additional visits on your journey to a healthier you, you may benefit from the Nutrition Package that includes the Assessment & 3 follow up visits. 

1-Month Nutrition Counseling Package

The 1-month nutrition counseling package includes an initial assessment will be conducted of your medical, dietary, and medication history, diagnostic and lab test results. Based on this information obtained, we will work with you to set goals and create a personalized plan to include lifestyle, meal planning based on  budget & taste preferences during the included 3 (45 minute) Nutrition follow ups 

Anti-Inflammatory/Mediterranean Diet Counseling

Dietary changes will help reduce inflammation levels in the body and restore normal immune function. A 90 minute initial consultation & 2 weekly 45 minute sessions are included with focus on personalized goals & achievable objectives to support making lasting changes to enhance overall health.

Core Food Plan (CFP)

The Core Food Plan (CFP) is designed for those who are interested in:

  • Core principles of healthy eating
  • Health maitenance
  • Disease prevention 
  • Awareness of one’s relationship with food

It takes the elements of the Mediterranean diet & the hunter-gatherer approach (referred to as “Paleo” diet) and can be easily adjusted to suit personal preferences and health needs.  It uses the principles of “food as medicine” which will support both you health goals and assist in improving your relationship with food.  

Low Fodmap Diet

Food is a common trigger of digestive symptoms.  Research has shown The Low FodMap Diet can aid with improving GI (gastrointestinal) symptoms (gas, bloating, change in bowel habits) related to IBS.  If you & your Physician or Healthcare Provider have determined that you may benefit from this dietary approach, please schedule your appointment which will include a full nutrition assessment and 3 nutrition follow ups scheduled appropriately to allow for integration of all three phases of the diet. 

This package will require a Medical Referral if Insurance is to be billed. 

Pre-DM & DM (Type 2) Meal Planning/Lifestye Intervention

If you are having challenges with meal planning/lifestyle changes  to support your desired goals in the management of your DM/Pre-DM, let us work with you individually to create an eating plan customized to fit your preferences along with resources that  will support you with long-term sustainment in your journey to a healthier you.  This package will include a 90 minute initial Nutrition Assessment & 3 (45 minute) follow up visits (adjusted per your Insurance Coverage).  

Maitenance Plan 1 visit per month copy

If you have already worked with Essential Nutrition for 1 month on any of our plans and want to cointinue working with us we offer Maitenacen Plan for continued suport and accountability. 

The Maitenance Plans is perfect for you in you are:

1) seeking cointinued nutrition support to hold you accountable as you continue your journey

2) want to maintain or continue your weight loss to your goal weight

3)  want to continue to improve your DM management

4) or need extra visits with your RD for any reason.  

Maitenance Plan  2 visits per month

If you have already worked with Essential Nutrition for 1 month on any of our plans and want to cointinue working with us we offer Maitenacen Plan for continued suport and accountability. 

The Maitenance Plans is perfect for you in you area:
